Check Your Pay Stub Benefits Deductions

Even if you didn't change your benefits during annual enrollment in November, paycheck deductions for benefits might have changed. Take a look at your first pay stub in 2019 to verify deductions for benefits coverage. Your online pay stub is available through the ADP Portal. The 2019 rate sheet showing contributions per pay period for medical, dental and vision coverage is available here.

Your 2019 benefit deductions may be higher or lower than last year, depending on your elections during the annual enrollment period and whether you qualified for the wellness discount on medical coverage. To verify your 2019 deductions, compare your first 2019 pay stub against the confirmation statement you printed during enrollment. Some of the items your pay stub will show include:

  • Pay period deductions for medical, dental, vision, flexible spending accounts and other coverage you elected or received for 2019.
  • Health Savings Account (HSA) contributions if you participate in the BCBSTX HSA Plan and elected an HSA contribution amount. 

If you didn't print a confirmation statement after you enrolled, you can see your 2019 benefits coverage any time at mybmcrewards.com. Navigate to Your Benefits Resources and select the Health and Insurance tab at the top of the page and then click on Current Coverage. You can also call Your Benefits Resources at 1-877-262-4849 if you have any questions about your benefits deductions. Representatives are available weekdays from 8 a.m. to 9 p.m., Eastern time.

Annual BMC Contribution Added to HSAs

If you are enrolled in the BCBSTX Health Savings Account (HSA) medical plan for 2019 and have an HSA with UMB Bank, BMC deposited 50% of the annual company-provided contribution to your account in January.

BMC’s contribution depends on your base salary and how many dependents you cover under the HSA medical plan. Fidelity recommends that you consider contributing the maximum you can afford to your HSA, up to your maximum. Read More

BMC will make a second contribution to your account in July for the remaining 50% of the annual amount listed above.

You are eligible for BMC's HSA contribution if you are actively employed by BMC on the date the company contribution is deposited to your HSA.

 You can check your balance and change your contributions on the My BMC Benefits & Compensation website, click on Your Benefits Resources and follow these steps to make the change. You can also call 1-877-262-4849 and select Your Spending Accounts. See the online Benefits Guide for details.

Tax Form Reminders

Medical Tax Form 1095

Form 1095 is a tax form required under the Affordable Care Act (ACA). This form confirms you and your covered dependents had BMC medical coverage during 2018. While Form 1095 is not required to file your taxes, you should maintain a copy for your records in the event of an audit.

How to view/print your Form 1095

Go to My Compensation & Benefits and click on Go to Your Benefits Resources > Medical Tax Form 1095 >View/Print Your 1095 to:

  • Sign up to receive an email or text message when your tax form is available in January.
  • Review your email or phone number for early notification.
  • Access your form by January 31 (if you signed up for early access).
  • View/Reprint your form after February 7 (if you didn't sign up for early access).

Health Savings Account Forms

  • Form 1099-SA – Reports distributions made from Health Savings Accounts. This form will be sent through regular mail. Request reprints by calling UMB at
    866-520-4472.
  • Form 5498-SA: This form is informational only to report your HSA contributions, which are also available on your W-2. This form is not required for filing taxes and will be sent through regular mail in May 2019.
