Step 1: Complete Your Well-Being Assessment

The survey asks simple questions about your health habits and your overall assessment of your well-being. Your “numbers”—like your weight, height and blood pressure—will be loaded into the Well-Being Assessment about two weeks after completing your onsite screening or sending in your at-home testing kit or physician form. When you finish, you’ll receive a plan for taking small steps to improve your health and well-being and avoid costly medical problems later.

Step 2: Get a Health Screening

To date, nearly 700 BMC employees have participated in onsite health screenings and more than 400 employees have ordered at home kits or doctor's forms. If you missed the onsite screening, you have until July 1, 2012, to order a home finger-stick kit or form for your doctor to complete.
